Abstract	In today’s digital healthcare environment, communication between doctors, nurses, and patients remains fragmented. Our research presents the design and implementation of a secure, centralized online portal to bridge this gap. The system allows user registration, role-based login, appointment booking, and session-based data control. Using PHP, MySQL, and HTML/CSS, the project ensures smooth interaction and record handling in a web-based environment. This paper demonstrates how such a system can improve accessibility, coordination, and patient satisfaction.
