Role of CRP and WBC in the Diagnosis of Acute Appendicitis: A Retrospective Study at a Military Hospital in North East India

| Abstract | Background: Acute appendicitis is a common surgical emergency. While ultrasonography (USG) is often used for diagnosis, inflammatory markers such as C-reactive protein (CRP) and white blood cell (WBC) count may assist in decision-making. Objective: To evaluate the diagnostic utility of CRP and WBC count in suspected cases of appendicitis diagnosed on USG. Methods: A retrospective study of 100 patients conducted at 188 Military Hospital (MH) over 5 years. All patients had USG suggestive of appendicitis and underwent blood tests. CRP and WBC values were analysed in relation to confirmed diagnosis based on surgery and histopathology. Results: At CRP ≥10 mg/L, sensitivity was 70% and specificity 32%. For WBC ≥11×10⁹/L, sensitivity was 60% and specificity 38%. ROC analysis showed poor discrimination (AUC ~0.5). Logistic regression did not reveal either CRP or WBC as statistically significant predictors. Conclusion: CRP and WBC alone are insufficiently accurate for diagnosing appendicitis.
