Development of AST and ARD templates for the electronic Medico-legal documentation

Abstract Medico-legal documentation plays a critical role in bridging healthcare delivery and the legal system. Traditional manual medico-legal records often suffer from incompleteness, illegibility, and delays, resulting in challenges for both healthcare providers and the judiciary. This study, conducted at Jai Prakash Narayan Apex Trauma Centre (JPNATC), AIIMS, aimed to analyze existing medico-legal documentation practices, perform a gap analysis, and propose standardized Assessment and Summary Template (AST) and Accident/Reporting Documentation (ARD) formats for electronic medico-legal documentation. Through process mapping, checklist-based gap analysis, and focused group discussions, deficiencies in documentation were identified and addressed in the design of AST and ARD templates. The study concludes that standardized electronic formats improve completeness, legibility, and legal robustness of medico-legal documentation, thereby enhancing efficiency and compliance
