Abstract Background: Trauma is a major cause of morbidity and mortality globally. This study evaluates the prognostic efficacy of serum lactate levels in predicting early outcomes among trauma patients.

Methods: A prospective study conducted on 122 trauma patients admitted to Amaltas Institute of Medical Sciences. Serum lactate levels were taken on admission, 6 hours, and 24 hours, and correlated with Injury Severity Score (ISS), ICU requirement, hospital stay, and mortality.

Results: Elevated lactate level were significantly associated with prolonged hospital stay, ICU admissions, and higher mortality. ROC analysis showed serum lactate (AUC=0.72) predicted mortality more accurately than ISS (AUC=0.68).
