Abstract Abdominal pregnancy (AP) is defined as the implantation and development of the fertilized egg in the peritoneal cavity. Abdominal pregnancy is relatively common in countries with low socio-economic levels. We describe here a rare case of abdominal pregnancy at 15 weeks with fetal demise, initially diagnosed by ultrasound and then confirmed by laparotomy. The challenges faced were primarily diagnostic due to the presence of a fibroid uterus, and secondarily therapeutic due to difficulties in extracting the placenta, which was adherent to neighboring structures. Adhering to certain guidelines could help avoid pitfalls in ultrasound diagnosis and placenta extraction in cases of abdominal pregnancy.
