Derotation techniques for Severely Rotated teeth: Conventional Biomechanical Approaches

| Abstract | Background: Severely rotated teeth, particularly maxillary premolars and canines, present significant challenges in orthodontic treatment due to their resistance to conventional alignment methods. Traditional approaches often involve the use of round NiTi wires during the leveling and aligning phase; however, these may be insufficient for correcting severe rotations. To address this, biomechanical strategies employing couple mechanics have been developed. These systems utilize two equal and opposite forces applied at different points to produce pure rotation without unwanted translational movement. An effective application of this principle involves the use of elastomeric chains (e-chains) connected from the rotated tooth to the molars, generating a couple that induces rotation. This method allows for precise and controlled derotation, facilitating the alignment of the tooth into its correct position. The application of light, continuous forces through such mechanics not only accelerates the derotation process but also minimizes the risk of adverse effects, such as root resorption or anchorage loss. Clinical cases have demonstrated the efficacy of this approach in achieving rapid and stable derotation of severely rotated teeth. These advancements underscore the importance of tailored biomechanical strategies in enhancing the predictability and efficiency of orthodontic treatments for complex cases. |
