| Abstract | Type 2 diabetes mellitus prevalence in the geriatric population continues to riseglobally, presenting unique therapeutic challenges due to age-related physiological changes, comorbidities, and polypharmacy, prompting a comprehensive literature review using PubMed, Cochrane Library, and EMBASE databases focusing on studies published between 2015-2024 involving geriatric patients with T2DM and oral antidiabetic medications to examine the efficacy, safety profiles, drug interactions, and evidence-based recommendations for clinical practice of oral hypoglycemic agents in this population. Current evidence supports individualized treatment approaches in geriatric patients, with metformin remaining first-line therapy when tolerated, while newer agents like DPP-4 inhibitors and SGLT-2 inhibitors show promise with improved safety profiles, and traditional agents like sulfonylureas require careful monitoring due to hypoglycemia risk, ultimately demonstrating that management of T2DM in geriatric patients requires a patient-centered approach that balances glycemic control with safety considerations and quality of life through regular monitoring and dose adjustments for optimal outcomes. |
