| Abstract | Unicystic ameloblastoma (UA) is a less aggressive yet locally invasive odontogenic tumor that typically affects the posterior mandible of younger individuals. Histologically, UA is classified into luminal, intraluminal, and mural subtypes, with the mural variant associated with a higher recurrence risk. While radical surgical resection ensures minimal recurrence, it can cause considerable postoperative morbidity. Hence, conservative treatment remains a viable option, especially in young patients. A 17-year-old female presented with a gradually enlarging, painless mandibular swelling and paraesthesia. Imaging revealed a multilocular radiolucent lesion extending from 43 to 48, associated with cortical expansion and root resorption. The lesion was treated by enucleation and peripheral ostectomy via an intraoral approach. A surgical stent was placed postoperatively to maintain hygiene, promote healing, and prevent dead space. The patient demonstrated satisfactory healing with no recurrence at the 18-months follow-up. This case highlights the effectiveness of conservative surgical management in unicystic ameloblastoma, preserving jaw function and aesthetics in a young patient. Adjunctive use of a surgical stent further optimized healing. |
