A Bird’s Eye View on Sama-Nirama conditions in Panchakarma: A Review

| Abstract | In Ayurveda, the most important part of the treatment and for maintaining optimal health is balancing Tridosha (Vata, Pitta and Kapha). Within the framework of Dosha, the concepts of Sama (with toxins) and Nirama (without toxins) play a crucial role in assessing the body’s physiological and pathological states. The presence or absence of Aama (a toxic by-product of incomplete digestion) determines whether a Dosha is Sama or Nirama. Aama which is a sticky, heavy, foul-smelling metabolized substance produced when the digestive fire (Agni) is weak or compromised can mix with the Dosha, leading to blockages in bodily channels (Srotas) and disrupting normal physiological functions. When a Dosha is mixed with Aama, it is referred to as a Sama Dosha and is typically associated with diseases and imbalances in the body. Sama Dosha, burdened by toxins, requires detoxification and cleansing. A Nirama Dosha is a state where the Dosha exists in its pure form, indicative of a healthier state and is free from Aama. In this state where Dosha are better managed through therapeutic interventions like balancing diets, lifestyle adjustments, and herbal remedies without the need for detoxification. The distinction between Sama and Nirama Dosha is a cornerstone of Ayurvedic diagnosis and treatment. Understanding this difference helps practitioners design more effective, individualized treatment plans, paving the way for optimal health and well-being. Present review article emphasizes consideration of Sama-Nirama Dosha-Dhatu-Mala in the context of Panchakarma. |
