Reliability of walker viewtm of instrumented treadmill in motor incomplete spinal cord injury subjects

Abstract Background: Individuals with SCI and clinicians have identified regaining walking ability or gait as a major priority in achieving independence due the benefits associated with standing and physiological walking. The analysis of gait for deviations is of importance for the individualized rehabilitation protocol. Objectives: The purpose of the study is to evaluate the test-retest reliability of walker view of instrumented treadmill in motor incomplete spinal cord injury subjects.
Material and Methods: 30 subjects with motor incomplete spinal cord injury, after meeting the inclusion and exclusion criteria were recruited. The subjects were explained about the study and the features of walker view. They were instructed to walk on the walker view at their pre-determined comfortable speed, the analysis report was generated. After 24-48 hours after, the test was repeated with the same speed and data was collected.
Results: Trunk flexion-extension (ICC 0.94), right (ICC 0.93), and left (ICC 0.95), step length, contact time right (ICC 0.94), and average step cycle (ICC 0.93), have excellent, knee flexion-extension right (ICC 0.83), and contact time left (ICC 0.81) good, trunk lateral flexion-extension (ICC 0.69), hip flexion-extension left (ICC 0.71), hip flexion-extension right (ICC 0.72), knee flexion-extension left (ICC 0.71), and vertical displacement of COG (ICC 0.73) have moderate, load symmetry (ICC 0.32) is the only parameter reported to have poor test-retest reliability.
The Bland-Altman plots were plotted for each parameter separately between the difference of readings from day 1 and day 2 and mean of differences. The plot showed high agreement with minimum number of outliers being 1 in trunk flexion-extension, average step cycle and maximum number of outliers being 4 in contact time left.
Conclusion: This study concludes that walker view of instrumented treadmill is reliable instrument to analyse gait in subjects with motor incomplete spinal cord injury, however, for load symmetry parameter with ICC: 0.32, it can not be considered as reliable.
