Abstract Background: Recurrent ankle sprains frequently result in chronic ankle instability (CAI), which is more common in athletes who play high-impact sports like football. Repeated micro-trauma and ligamentous laxity can significantly affect athletic performance, balance, and joint proprioception. This study aimed to determine the prevalence of CAI among football athletes aged 18–26 years using the Cumberland Ankle Instability Tool (CAIT).

Methods: A cross-sectional study was conducted among 87 recreational football athletes (66 males and 21 females) recruited through convenient sampling. Participants completed the CAIT questionnaire, and scores ≤24 were used to identify CAI. Data were analysed using SPSS version 26 with descriptive statistics and Spearman’s rank correlation.

Results: The mean CAIT scores were 23.5 ± 4.47 (right ankle) and 23.8 ± 4.62 (left ankle). CAI is highly prevalent in football athlete population. No significant correlations were observed between CAIT scores and training experience or training duration, indicating that instability was independent of basic training exposure. The strong relationship between right and left CAIT scores showed that players who have instability in one ankle often experience it in the other as well, indicating consistent patterns of ankle weakness.

Conclusion: Chronic ankle instability is highly prevalent among football athletes, with females exhibiting a slightly higher occurrence. The findings emphasize the need for early screening, sex-specific preventive strategies, and structured rehabilitation programs focusing on balance, proprioception, and neuromuscular control to reduce recurrence and enhance performance.
