Effect of Fast Muscle Activation and Stepping Training (Fast) on Balance and Mobility in Individuals with Cerebellar Ataxia-An Experimental Study

Abstract ABSTRACT
INTRODUCTION: Cerebellar ataxia is heterogeneous group of movement disorder caused by damage of cerebellum. Balance impairment is key issue in 70% in individuals with cerebellar ataxia, yet targeted training of movement speed required to regain balance has been largely overlooked. The Fast muscle Activation and Stepping Training (FAST) program incorporates rapid functional movements that produce bursts of muscle activation essential for effective stepping, balance and postural recovery
AIM OF STUDY: This study aim to evaluate the effectiveness of Fast Muscle Activation and Stepping Training on balance and mobility in individuals with cerebellar ataxia.
METHOD: A pre-post experimental design was used. 24 individuals with cerebellar ataxia (aged 40-70 year, both male-female) diagnosed by neurologist and willing to participate were recruited through random sampling. Individuals with BBS score (21-40) moderate balance impairment, Time up-Go (> 20 second) moderate risk fall, SARA (6-23) moderate ataxia, MMSE >24, can ambulate independently with/without cane were included. Individuals with sensory ataxia, prosthetic device, any congenital anomalies, spine /LL injuries affecting weight bearing, history of psychological or other neurological/cardiorespiratory conditions, peripheral neuropathy, and seizures were excluded. Individuals underwent intervention for 3 days/week for 8 weeks (45 minutes/session). Outcomes used were BBS, SARA, TUG to measure balance and mobility. Data were analyzed using SPSS version 20.0. Descriptive statistics summarized baseline characteristics. Shapiro-wilk test applied for normality and Paired t-test used to compare Pre-Post intervention.
RESULT: Shapiro-wilk test confirmed normality distribution (p>0.05). Paired t-test analysis revealed statistically significant improvement in balance and mobility post-intervention (p<0.05)
CONCLUSION: The FAST intervention demonstrated significant improvement in balance and mobility in cerebellar ataxia. It is considered feasible for clinical application in individuals with cerebellar ataxia.
