Reviving Speech and Balance: An Ayurvedic Management of Multiple System Atropy: A Case Report

| Abstract | Multiple System Atrophy (MSA) is a rare, progressive neurodegenerative disorder characterized by motor abnormalities including akinesia, rigidity and postural instability, accompanied by autonomic and cerebellar dysfunction. Modern management remains largely symptomatic, emphasizing the need for alternative approaches that can improve quality of life and functional stability. There is no direct diagnostic correlation to MSA in Ayurveda, however it can be compared to Vatavyadhi. Objective: To evaluate the efficacy of Ayurvedic Panchakarma interventions and internal medications in the management of a clinically diagnosed case of Multiple System Atrophy. Methods: A diagnosed case of MSA-C with dysarthria, ataxic gait, rigidity, and imbalance was treated through a comprehensive Ayurvedic approach involving Abhyanga, Parisheka, Jambira Pinda sweda, Shirodhara, Nasya, Basti Chikitsa, and selected Rasayana formulations. The treatment was planned based on Vata-Kapha predominance and Majja Dhatu Kshaya pathology. Results: Marked improvement was observed in balance, speech clarity, and motor coordination. The Unified Multiple System Atrophy Rating Scale (UMSARS) Part IV Global Disability Grade improved from Grade 3 (before treatment) to Grade 1 (after treatment), indicating significant recovery and improved daily activity performance. Conclusion: The integrative Ayurvedic approach focusing on Vata-Kapha pacification, Majja Dhatu Poshana, and Manovaha Srotoshodhana demonstrated promising outcomes in MSA management. This highlights the scope for further clinical and experimental studies to validate Ayurvedic modalities in neurodegenerative conditions. |
