| Abstract | kāla (Time) is a fundamental principle in Ayurveda, described as the driver of transformation in nature and life. While its philosophical and seasonal aspects are widely studied, structured documentation of its clinical applications remains limited. This article aims to critically analyze the role of kāla in Ayurvedic clinical practice, integrating classical textual evidence with practical therapeutic applications. A narrative review of caraka saṁhitā, suśruta saṁhitā, kaśyapa saṁhitā, aṣṭāṁga hr̥daya, and traditional commentaries was conducted. Relevant passages on kāla in relation to disease dynamics, doṣa variation, treatment timing, and seasonal regimens were extracted. Data were organized thematically into classifications of kāla, time-based doṣa physiology, disease-stage therapeutics, maṇḍala-based treatments, and Pañcakarma scheduling. kāla operates on multiple clinical levels — philosophical (eternal and cyclical), seasonal (R̥tu), diurnal, life-stage, and disease-stage. Applications include R̥tu-based doṣa variation, stage-specific treatment protocols, maṇḍala-based formulations, and optimal Pañcakarma timing. Systematic incorporation of kāla into Ayurvedic practice optimizes therapeutic efficacy, reduces adverse effects, and harmonizes treatment with biological rhythms — paralleling the principles of modern chronotherapy. |
