Case Report: Multimorbidity in a Patient with CKD, Sepsis, Bilateral Pneumonitis, CVA, and Hypocalcaemia

| Abstract | Multimorbidity among the elderly presents an increasing clinical challenge, especially when it includes critical illnesses such as chronic kidney disease (CKD), sepsis, bilateral pneumonia, cerebrovascular accidents (CVA), and hypocalcemia. We report on an 85-year-old man with a known history of CKD and chronic obstructive pulmonary disease (COPD), who was hospitalized due to decreased oral intake, altered mental status, and respiratory distress. Clinical assessments and investigations indicated acute-on-chronic kidney injury, sepsis resulting from bilateral pneumonia, hypocalcemia, and neuroimaging suggesting chronic ischemic stroke. Laboratory results indicated leukocytosis, increased serum urea and creatinine levels, and significant electrolyte imbalances. CT scans of the brain displayed multiple chronic lacunar infarcts along with evidence of cerebral atrophy. The patient received treatment with IV antibiotics, renal support, electrolyte management, enteral nutrition, and respiratory therapy for support. Although his vital signs stabilized, his neurological recovery was constrained. This case illustrates the challenges in caring for elderly patients with multiple systemic conditions and emphasizes the need for timely, multidisciplinary approaches to improve outcomes in cases of advanced multimorbidity. |
