Heterochromatic Variations in Cataract with Special Refrence to Timira

Abstract Cataract, a leading cause of preventable blindness worldwide, presents with diverse chromatic variations that reflect both the stage of lens opacification and underlying systemic or ocular pathology. Timira is a disease affecting dristi, explained in Ayurveda classics as a progressive condition that results in the colouration of dristi and loss of vision in later stage. Progressive stages of cataracts are correlated to different stages of Timira-Kacha-Linganasha based on their signs and the resultant colour of the lens. In addition, by applying the concept of guna and the specific functions of dosha on the pathophysiology of different types of cataracts, an attempt is made to better understand the stages of cataracts and help plan the management.
Haritha Samhita & sushrutha Samhita mentions colour changes (varna vikriti) with specific dosha predominance. Modern ophthalmology similarly recognises colour gradations in nuclear sclerosis, cortical cataracts, and rare varieties such as cerulean, snowflake, and Christmas tree cataracts, each with distinct etiopathogenesis and clinical significance. This article integrates Ayurvedic and modern perspectives to classify and interpret heterochromatic lens changes, analyse their pathophysiological basis, and highlight their diagnostic, prognostic, and therapeutic implications. Emphasis is placed on correlating colour patterns with patala involvement, disease stage, and surgical planning.
