Casein Phosphopeptide–Amorphous Calcium Phosphate (CPP–ACP) in Pediatric Dentistry — A narrative Review

Abstract Casein phosphopeptide–amorphous calcium phosphate (CPP–ACP) is a milk-derived nanocomplex that stabilizes bioavailable calcium and phosphate ions next to tooth surfaces and promotes enamel remineralization. Over the past two decades, CPP–ACP formulations (e.g., GC Tooth Mousse, MI Paste) have been evaluated for caries prevention, management of white-spot lesions (WSLs), erosion prevention, and treatment adjuncts in special-needs pediatric populations. Clinical and laboratory evidence suggests beneficial effects, particularly as an adjunct to fluoride and for certain high-risk pediatric groups; however, heterogeneity in study design, small sample sizes, and variable delivery protocols limit definitive conclusions. Safety concerns are limited to milk-protein allergy. Future research should prioritize large randomized pediatric trials with standardized outcomes and head-to-head comparisons against modern fluoride regimens.
