Empyema Caused by Pasteurella multocida Complicated with Sepsis and Acute Respiratory Failure: A Case Report

| Abstract | This case report describes a rare presentation of Pasteurella multocida empyema complicated by sepsis and acute respiratory failure in an elderly patient with underlying comorbidities. The patient presented with dyspnea and was found to have a large right-sided pleural effusion. Pleural fluid analysis revealed purulent exudate with neutrophil predominance, positive Rivalta test, elevated protein, and low glucose levels, confirming the diagnosis of empyema. Culture of the pleural fluid identified P. multocida, a zoonotic pathogen commonly associated with animal exposure. The patient developed sepsis and type I respiratory failure requiring endotracheal intubation, mechanical ventilation, and hemodynamic support. Management included broad-spectrum antibiotic therapy, pleural drainage, and intensive care monitoring. This case highlights the importance of early recognition, appropriate antimicrobial therapy, and multidisciplinary management, particularly from an anesthesiology and critical care perspective, to prevent progression to multi-organ failure and improve clinical outcomes in patients with empyema-associated sepsis. |
