Effectiveness of Acbt and Goldfish Exercise on Respiratory Function in Male Head and Neck Cancer Patients

Abstract Background: Over 200,000 cases of head and neck cancer (HNC) occur each year in India versus 30,000 for the USA. The most common postoperative oral problems occurring after radiation and chemotherapy are difficulties in swallowing, trismus, dysarthria, and other complications include injury to the phrenic nerve causing paralysis to the ipsilateral diaphragm.
Objective: To evaluate the effectiveness of active cycle of breathing techniques (ACBT) and goldfish exercise in a patient with head and neck cancer.
Methods: A Case-series of 8 Head and Neck Cancer male patient with complaints of breathlessness, difficulty in opening mouth, swallowing and reduced chest expansion were included in the study on their consent for participation. Active cycle of breathing techniques and goldfish exercise was given in six consecutive weekly sessions for four weeks each of 30 min duration. Outcome measures are MMRC, Mouth opening range, chest expansion and FACT-H&N Questionnaire.
Results: Data were analysed using SPSS 23.0. MMRC scores improved from 3.50 ± 0.53 to 0.38 ± 0.52 (z = 2.714). Mouth opening increased from 20.25 ± 4.68 mm to 31.88 ± 5.30 mm (z = 2.565). FACT-H&N scores rose from 55.50 ± 6.74 to 112.38 ± 14.18 (z = 2.521). All improvements were statistically significant (p < 0.05).
Conclusions: The combined intervention significantly improved respiratory function, mouth opening, and quality of life, with all participants showing over 50% improvement in MMRC, mouth opening and FACT-H&N scores. These findings support integrating ACBT and goldfish exercises into rehabilitation programs for HNC patients.
