Effect of Lumbosacral Chiropractic Manipulation and Hamstring Muscle Energy Technique on Chronic Knee Pain: A Randomized Clinical Trial

| Abstract | Background: Chronic knee pain is one of the most common musculoskeletal problems and may be influenced by dysfunction in the lumbosacral region and tightness in the hamstrings. Chiropractic treatment and Muscle Energy Technique (MET) are manual therapy interventions aimed at improving joint and muscle function. Objectives: To evaluate the combined effect of lumbosacral chiropractic treatment and hamstring MET on pain, range of motion, and function in patients with knee pain. Methods: Sixty patients aged 25–60 years with chronic nonspecific knee pain were randomly allocated into two groups. Group A received conventional physiotherapy, while Group B received conventional physiotherapy plus lumbosacral chiropractic manipulation and hamstring MET for 4 weeks. Outcome measures were Visual Analogue Scale (VAS), Knee Range of Motion (ROM), Active Knee Extension (AKE) Test, and Knee Injury and Osteoarthritis Outcome Score (KOOS). Results: Both groups showed significant improvement (p<0.05), but the experimental group showed superior improvement in all parameters. VAS: 6.9 ± 1.4 → 2.8 ± 0.9 (p<0.001); AKE: 137.4° ± 6.1 → 151.2° ± 4.8 (p<0.001); KOOS: 57.6 ± 8.9 → 76.1 ± 6.8 (p<0.001). Conclusion: The combination of lumbosacral chiropractic manipulation and hamstring MET significantly improves pain, flexibility, and functional outcomes compared to conventional therapy alone. |
