A Descriptive Study On Prescription Pattern And Medication Adherence In Patients Suffering From Lichenoid Eruption

| Abstract | Abstract Introduction Lichenoid eruption refers to a skin condition characterized by papular or plaque-like rashes that resemble lichen planus. Lichenoid drug eruptions are most commonly observed on the skin, while mucosal involvement is relatively rare. However, adults are more prone to oral manifestations, termed oral lichenoid drug eruptions. Lichen planus (LP) is an inflammatory disease of the skin and mucous membranes with no known etiology. Globally, cutaneous LP affects approximately 0.2%–1% of adults, while oral lichen planus (OLP) is reported in 1%–4% of the population. Women are affected 1.5 times more frequently than men, and the condition typically presents between 30 and 60 years of age. LP in children is uncommon, comprising less than 5% of cases. Clinically, LP manifests as purple, itchy papules that may appear anywhere on the body. On mucosal surfaces, such as the oral cavity, it presents as white patches that may sometimes progress to painful ulcers. OLP, also known as oral lichen planus, is a chronic mucocutaneous immunological disorder with significant impact on patient comfort and quality of life. Objectives: To describe the prescription pattern in patients diagnosed with lichenoid eruption. To evaluate medication adherence among patients with lichenoid eruption. Methodology An observational descriptive study was conducted among 70 patients attending the Out-Patient Department of Dermatology, ESIC MC–PGIMSR & Model Hospital, Rajajinagar, Bengaluru. Data were collected using a self-designed patient profile form that included demographic details, clinical presentation, diagnosis, comorbidities, and treatment patterns. Medication adherence was assessed using the Morisky Medication Adherence Scale (MMAS-4). All collected information was entered and analysed using Microsoft Excel for descriptive statistics. Results: Out of 34 subjects who met the inclusion criteria, 62% were females and 38% were males. The majority of patients were in the 36–44 and 45–54 years age groups. A total of 116 drugs were prescribed, most of them in generic names. Corticosteroids were the most frequently prescribed medications (100%, n=34). Based on MMAS-4 scores, 16 patients showed good adherence, 10 had moderate adherence, and 8 had poor adherence. Conclusion: Lichenoid eruptions predominantly affect middle-aged individuals, with a higher prevalence among women. Corticosteroids remain the mainstay of treatment, often supplemented by antihistamines and moisturizing agents to alleviate symptoms. Most patients demonstrated good medication adherence, which plays a crucial role in improving therapeutic outcomes. Enhancing patient education and follow-up support can further strengthen adherence and overall quality of life in affected individuals. |
