Integrated Cardiorespiratory–Musculoskeletal Telerehabilitation for Complex Multisystem Impairments: A Professional Review

The increasing prevalence of patients presenting with concurrent cardiorespiratory and musculoskeletal impairments has highlighted the need for integrated, technology-enabled physiotherapy models. Telerehabilitation has emerged as a viable solution that ensures continuity of care, promotes functional independence, and maintains clinical safety when in-person access is limited. This professional review synthesizes current evidence on physiological interactions, remote assessment standards, exercise prescription strategies, and monitoring protocols in cardio–musculoskeletal telerehabilitation.

Patients with COPD, post-COVID syndrome, cardiac dysfunction, osteoarthritis, or chronic pain often experience compounded dyspnea, reduced functional capacity, muscle deconditioning, and movement restrictions. Integrating aerobic conditioning, respiratory muscle training, postural correction, and musculoskeletal strengthening through virtual platforms provides a holistic treatment approach. The review outlines a structured protocol that includes pre-session safety screening, technology readiness checks, individualized exercise dosing, and real-time telemonitoring using validated tools such as the Borg RPE, dyspnea scales, and tele-observed functional tests.

Outcome measures discussed include the Six-Minute Walk Test, Two-Minute Step Test, Sit-to-Stand Test, remote ROM analysis, and pain/function indices. Evidence demonstrates improvements in exercise tolerance, dyspnea scores, adherence, and musculoskeletal function within hybrid or fully virtual programs.

This review concludes that integrated cardio–MSK telerehabilitation is a clinically sound and scalable model for managing multisystem impairments. With adherence to professional guidelines and robust monitoring frameworks, physiotherapists can deliver safe, effective, and patient-centered care remotely, making it a valuable component of modern rehabilitation practice.
