Ayurvedic Management of Karnastrava: A Case Study With Tribhuvan Kirti Ras and Vidangadi Karnadhoopan

| Abstract | Abstract Introduction Shalakya Tantra, a specialized branch of Ashtanga Ayurveda, addresses disorders of the head and sensory organs, including 28 classified ear diseases (Karna Roga). Among these, Karna Strava—characterized by persistent ear discharge—correlates with Acute Suppurative Otitis Media (ASOM) in modern medicine. Conventional treatments often involve pharmacological therapy and surgical intervention, yet recurrence of discharge and associated hearing loss remain significant challenges. Ayurvedic literature, particularly the Sushruta Samhita, outlines holistic management strategies for Karna Strava, including Shirovirechana, Karna Dhoopan, Karna Puran, and Karna Dhavan. Of these, Karna Dhoopan—a medicated herbal fumigation technique—is emphasized as the treatment of choice. This study explores the efficacy of Karna Dhoopan in reducing ear discharge and preventing recurrence, offering a potential integrative approach to managing ASOM through Ayurvedic principles. Objective: The primary objective of this study is to evaluate the efficacy and safety of vidangadi karnadhoopan and Tribhuvankirti ras with honey in the management of Karnastrava. Method: A 57-year-old male patient visited the ENT outpatient department at the ShalakyaTantra unit of Mansarovar Ayurvedic Medical College Hospital and Research Centre in Bhopal (M.P.). We started the Dhoopana karma as well as oral medication for 7 days. Result: The patient's clinical status was thoroughly assessed on the 3rd, 7th, and 15th day. It revealed a clear improvement in subjective parameters before and after the treatment, which strongly indicates a positive impact on the patient's condition. The treatment's effectiveness in this case study undeniably demonstrated highly satisfactory results. Discussion & Conclusion: In the present study Dhoopana karma and above-mentioned oral medication is an effective and safe therapeutic procedure in the management of Karnastrava.
