A Comprehensive Ayurvedic Approach to the Management of Koshta Shakashritha Kamala (Hepatocellular Jaundice) through Shamana Chikitsa: A Case Study

Abstract Introduction-Hepatocellular Jaundice is a clinical manifestation of disorders of underlying bilirubin metabolism, hepatocellular dysfunction, or biliary obstruction. Liver diseases contribute to 23.59% of mortality in the world and Ranks 27th as a major cause of death in world. Hepato cellular Jaundice is characterised by dark urine, pale stools, abdominal pain, itching, weight loss, fever with rigors, dry eyes/mouth and fatigue. It can be interpreted as Koshta Shakashritha Kamala, in Ayurveda w.s.r. aetiology and symptomatology. Koshta Shakashritha Kamala, is Raktapradoshaja vyadhi where dushita pitta and rakta leads to haridra netra, mutra, tvacha, nakha, daha (Yellowish discoluration of eyes, urine, skin, nails, burning sensation),Dourbalya(Weakness), Sadana(Prostration), Aruchi(Anorexia) and Karshata(Emaciation).
Materials and methods: A 46year female subject presenting with complaints of haridra netra (Yellowish discoluration of eyes), haridra twak (Yellowish discoluration of skin), haridra mutra (Yellowish discoluration of urine), kandu (itching all over the body), hrullasa (Nausea), bhrama (Giddiness), dourbalya (Excessive generalised weakness) since 2 months, associated with Adhmana (Bloating of abdomen), udarashoola (pain in abdomen), shuskaasya (Dryness of mouth at night time), shotha (B/l pedal oedema) since15 days approached Kayachikitsa OPD. She was diagnosed as kamala on the basis of clinical findings and advised investigations which revealed raised LFT values. USG abdomen showed Chronic parenchymal liver disease with Hepatomegaly. Subject was treated with Shamana aushadi on OPD basis.
Result: After the Shamana aushadi for 1 month in alternative follow up, symptomatic improvement along with substantial reduction in levels of LFT and improved quality of life in the subject was noted.

Discussion: The treatment protocol adopted includes shamana aushadhi as described in kamala chikitsa by considering the Agni, Rogibala, Dosha and Rogaavasta. Shamanoushadi such as Arogyavardhini vati, Kumaryasava, Gandhaka rasayana which was prescribed has antioxidant, hepatoprotective and anti-inflammamatory properties. Thus, were effective in relieving signs and symptoms of Koshta Shakashritha Kamala.
