Abstract Obesity is a major risk factor for NAFLD, and ALT is a key marker of liver injury. This cross-sectional study at Holy Care Medical Centre, Palakkad, assessed 295 adults (30–70 years) to compare deranged ALT levels between obese and non-obese groups. ALT was measured using the MISPA Nano Plus analyzer. BMI showed a significant positive correlation with ALT (r = 0.303, p < 0.001), with Obese II individuals displaying the highest median ALT (39 IU/L). Regression analysis revealed that higher meal frequency (p = 0.020) and sedentary behavior (p = 0.016) significantly contributed to elevated ALT. Overall, increasing BMI and unhealthy lifestyle factors were strongly linked to rising ALT levels, underscoring the importance of early screening and lifestyle modification to prevent liver dysfunction.
