Comparative Analysis of Fructosamine and Glycosylated Hemoglobin in Diabetic Patients with Polycythemia

Abstract Background: The standard biomarker for long-term glycemic control in diabetes mellitus is Glycosylated Hemoglobin (HbA1c). However, in conditions like polycythemia, due to altered red blood cell (RBC) turnover, its accuracy can be compromised. Fructosamine, a marker of short-term glycemic control, may serve as a more reliable alternative in such cases.
Objective: This study aimed to evaluate the reliability of fructosamine compared to HbA1c in diabetic patients with varying degrees of polycythemia and to assess the impact of RBC parameters on HbA1c levels.
Methods: A prospective comparative study was conducted at Padmashree Diagnostic Centre, Bangalore, over six months, involving 28 type 2 diabetic patients. Participants were categorized into three groups: normal RBC count (control), mild polycythemia, and moderate polycythemia. By using HPLC (D-10 Analyzer), Hemoglobin A1c levels were measured, and using a colorimetric method, fructosamine levels were measured. Correlations included in the Statistical analysis are ANOVA and Pearson.
Results: HbA1c and fructosamine levels showed a strong positive correlation in the control (r = 0.7) and mild polycythemia (r = 0.8) groups, while the correlation was moderate in the moderate polycythemia group (r = 0.6). Statistically significant difference was not observed in HbA1c or fructosamine values among the groups (p > 0.05), but in the polycythemia group, Hb levels and RBC counts were significantly elevated (p < 0.005).
Conclusion: In patients with polycythemia, HbA1c measurements may be unreliable due to altered RBC lifespan. Fructosamine offers a valuable alternative for monitoring glycemic control, especially in cases of elevated RBC counts. With larger associates, including severe polycythemia cases, further studies are recommended to confirm these findings.
