RESTORING NECK MOBILITY USING PNF STRETCHING IN POST-BURN CONTRACTURE: CASE STUDY

| Abstract | Background :- Post-burn cervical contracture is an extreme musculoskeletal adverse event due to excess scar tissue, which causes limited movement of the neck, pain, postural defect, and limited functions of routine activities. Traditional rehabilitation methods are mainly based on the use of the static stretching, positioning, and splinting but there is often a lack of functional recovery. Neurophysiological theories of muscle inhibition and facilitation are the foundation of Proprioceptive Neuromuscular Facilitation (PNF) stretching that has been shown to be most effective in enhancing flexion and range of motion but has only recently been used in cervical contracture following burns. Case History: -This is a male patient aged 21 years whose cervical contracture is post-burn in nature with limited cervical flexion, extension, lateral flexion and cervical rotation accompanied with severe pain and functional impairment. Clinical evaluation showed that scar tissue was evident, there was lower tissue elasticity as well as a significant loss of cervical range of motion in all positions that significantly impaired daily activity and quality of life. Intervention:The patient received a structured Physiotherapy program involving Proprioceptive Neuromuscular Facilitation stretching exercises, Hold-Relax and Contract-Relax-Agonist-Contraction being the main ones, and focusing on cervical flexion, and extension, lateral flexion, and rotation. The intervention was provided in a four weeks period, and scar management, postural, strengthening exercises, and a home exercise program structured to prevent recurrence of the contracture were provided. Findings:The post intervention revealed that the intensity of pain had reduced significantly with the Visual Analog Scale scores increasing to 4. The range of motion in the cervix was found to be significantly improved in all directions (flexion 7deg to 10deg), extension (9deg to 11deg), lateral flexion (13deg to 18deg) and rotation (15deg to 20deg). There was increased functional neck mobility, which allowed a greater number of activities of daily living. Conclusion: Proprioceptive Neuromuscular Facilitation stretching has been demonstrated to be effective in the restoration of cervical mobility, pain reduction, and functional outcome improvement in a patient with post-burn neck contracture as highlighted in this case study. PNF stretching seems to be a worthy addition to conventional burn rehabilitations, with better neuromuscular and functional advantages. More comprehensive studies are suggested in future to develop uniform PNF rehabilitation practice in treatment of post-burn cervical contracture.
