Rehabilitation after ring finger middle phalanx base fracture with pip joint subluxation a single patient report using soft tissue techniques with functional therapy: Case Study

| Abstract | Abstract Background: Due to the complex anatomy and the importance of the joint to the functioning of the hand, the proximal interphalangeal (PIP) joint injuries represent a clinical issue. Without proper and immediate rehabilitation, fractures of the ring finger phalanx base with a subsequent PIP joint subluxation often lead to pain, stiffness, instability and functional defects. Presentation of a Case: In this case study, the patient is a 27year old male, with PIP joint subluxation and ring finger phalanx base fracture in the right ring finger. Surgical treatment included open reduction, internal fixation using a K-wire, volar plate and collateral ligament repair. Intervention: The patient enrolled in a phase-by-phase physiotherapy rehabilitation program, which was done in a structured way during the period of April to September after surgery. The program emphasized early controlled mobilization, methods of working with soft tissues, joint mobilization, progressive strengthening, training of functional tasks, educating patients, and an exercise program at home. Outcome measure: Pain was measured with Numerical Pain Rating Scale (NPRS), whereas the range of motion at the PIP joint, Manual Muscle Testing (MMT), grip strength, and total functional hand use were measured as the functional recovery. Result: Once the patient was provided with rehabilitation, their pain management was considerably reduced, PIP joint mobility was considerably better, their fingers were stronger, their grip and hand dexterity were enhanced, and their daily and occupational activities became independent. Conclusion: The importance of tailored rehabilitation methods is highlighted in this case by showing the effectiveness of an early, systematic, and multimodal rehabilitation approach to physiotherapy as a method of maximizing functional recovery following surgical repair of severe damage to the PIP joints. |
