Non-Steroidal Management of Optic Neuritis in a Diabetic Patient through Ayurvedic Treatments. A Case Report

| Abstract | Abstract Background: Optic neuritis (ON) is an acute inflammatory demyelinating disorder of the optic nerve, commonly associated with autoimmune conditions, infections, or idiopathic causes. Conventional management primarily includes systemic corticosteroids; however, residual visual deficits may persist. In Ayurveda optic neuritis can be considered as pitta and raktaja vikara where sodhana, Rasāyana and Netra-Kriyākalpas can be performed. Case Presentation: A 46-year-old male with type-2 diabetes mellitus for 5 years presented with reduced vision in the right eye for 1 month. Best-corrected visual acuity was 6/12 (RE) and 6/6 (LE). Visual field testing showed an inferior field defect in the right eye. OCT revealed findings consistent with optic neuritis, and 3-D disc topography displayed disc edema. The patient underwent three cycles of Panchakarma and netra kriyakalpas over 6 months, including Nasya, Sadyovirechana, Kṣāra Basti, Netra Seka, Tarpana, Putapāka, along with internal medications including Guduchi Churna, Tamra Bhasma, Kamadugha Mukta, Mṛtyuñjaya Rasa, Kishora Guggulu, Amṛtottara Kaṣāya, and Lashuna Rasayana. Results: Significant improvement was documented in follow-up OCT, disc photography, and subjective visual clarity. Disc edema reduced markedly, and visual field defect showed improvement. No adverse effects were observed. Conclusion: This case demonstrates the potential therapeutic value of Ayurveda treatment in managing optic neuritis, particularly for reducing optic disc edema and improving visual functions |
