ROLE OF BASTI THERAPY IN AVASCULAR NECROSIS: AN AYURVEDIC APPROACH

Abstract Avascular necrosis (AVN) is a progressive degenerative disorder characterized by compromised blood supply to bone tissue, most commonly affecting the femoral head, leading to pain, restricted movement, and eventual joint collapse. From an Ayurvedic perspective, AVN can be correlated with Asthi–Majja Kshaya, Vata Vyadhi and Gambhira Vatarakta, where aggravated Vata Dosha and Dhatu Kshaya play a central role in disease pathogenesis. Among Panchakarma therapies, Basti Chikitsa is considered the most effective treatment for Vata-dominant disorders and is described as Ardha Chikitsa in classical texts.
Basti therapy acts primarily on Pakvashaya, the principal seat of Vata, thereby regulating systemic Vata functions. Therapeutic basti formulations such as Tikta Ksheera Basti, Niruha Basti, Anuvasana Basti and Yapana Basti provide both Shodhana and Brimhana effects, facilitating Srotoshodhana, improving microcirculation, and nourishing Asthi and Majja Dhatu. The use of Tikta Dravyas, Sneha and Ksheera in basti formulations helps in reducing pain, stiffness and inflammation while promoting tissue regeneration.
Basti therapy, when administered judiciously along with supportive internal medications and lifestyle modifications, may slow disease progression, improve functional outcomes and delay surgical intervention. Thus, Basti Chikitsa offers a holistic and effective Ayurvedic approach in the management of avascular necrosis.
