Abstract Introduction: Attention Deficit Hyperactivity Disorder (ADHD) is one of the most common neurobehavioral disorders of childhood, characterized by inattention, hyperactivity, and impulsivity, resulting in significant academic, behavioral , and social difficulties. Although conventional pharmacological therapies are effective, their long-term use in children is often associated with adverse effects, highlighting the need for safer and holistic treatment options. Ayurveda conceptualizes ADHD under the spectrum of Unmada, wherein vitiation of Vata Dosha and Manas leads to impairment of Dhee, Dhriti, and Smriti. Chatushkuvalaya Ghrita, a classical Medhya Rasayana formulation described in Ashtanga Hṛdaya, contains Nilotpala and Suvarna Patra, processed with Ghrita and Kshira, and is traditionally indicated for the enhancement of intellect, memory, and cognitive stability. Naladadi Ghrita, a well-established Medhya formulation, is commonly used as a standard comparator in pediatric cognitive disorders. The present randomized controlled trial aims to evaluate the efficacy of Chatushkuvalaya Ghrita in the management of ADHD in children and to compare its therapeutic effectiveness with Naladadi Ghrita. Materials and Methods: This interventional, prospective, open-label randomized controlled clinical trial will be conducted on 30 children aged 6–12 years diagnosed with ADHD based on DSM-5-TR criteria. Eligible participants will be randomly allocated into two groups of 15 each. Trial Group: Group A (Trial group) will receive Chatushkuvalaya Ghrita orally in a dose of 5 g twice daily before food with warm water for a duration of 30 days. Control Group: Group B (Control group) will receive Naladadi Ghrita orally in a dose of 5 g twice daily before food with warm water for a duration of 30 days. Conclusion: This study is designed to scientifically assess whether Chatushkuvalaya Ghrita is effective in reducing the core symptoms of ADHD in children and whether it demonstrates superior or comparable efficacy to Naladadi Ghrita. The findings may contribute evidence toward establishing Chatushkuvalaya Ghrita as a safe and effective Ayurvedic intervention in the integrative management of childhood ADHD.
