Functional Outcome in a Case of Atrophic Non-union of the Distal Radius Following Osteomyelitis: A Case Report

| Abstract | Abstract Purpose: Atrophic non-union of the distal radius following osteomyelitis is a rare and challenging condition, often resulting in significant functional impairment. This case report aims to present the surgical management and functional outcome of such a complex case, highlighting key strategies in restoring wrist stability and function. Methods: A 12-year-old male presented with pain, deformity, and restricted motion in the left wrist, one year after treatment for distal radius osteomyelitis. Radiographs and CT scans confirmed atrophic non-union of the distal radius. Surgical intervention included debridement of necrotic tissue, bone grafting from the fibula, and internal fixation using a plate and screw. Postoperatively, the patient was followed up clinically and radiographically over a period of 12 months, and functional outcomes were assessed using the Disabilities of the Arm, Shoulder and Hand (DASH) score and wrist range of motion. Results: Union was achieved at 16 weeks post-surgery. At the final follow-up, the patient demonstrated significant improvement in pain relief, grip strength, and wrist mobility. The DASH score improved from 72 preoperatively to 48 at one year. Radiographs showed satisfactory alignment and incorporation of the bone graft. Conclusion: Reconstruction of atrophic non-union of the distal radius after osteomyelitis can yield favourable functional outcomes with meticulous surgical planning, adequate debridement, structural bone grafting, and stable internal fixation. Early rehabilitation is essential to optimize recovery and restore wrist function in such complex cases. |
