Abstract Chondromalacia patella is a common cause of anterior knee pain in young and physically active individuals and is frequently associated with quadriceps weakness, patellar maltracking, and functional limitations. Stable Grade II meniscal tears are commonly managed conservatively through physiotherapy interventions aimed at reducing pain and improving joint function. A 21-year-old male presented with anterior knee pain aggravated during stair climbing, prolonged sitting, and repetitive knee movements. Clinical examination revealed quadriceps weakness, particularly of the vastus medialis oblique, tenderness around the patellofemoral region, and reduced functional ability. MRI findings confirmed chondromalacia patella associated with a Grade II meniscal tear. The patient underwent a 12-week physiotherapy rehabilitation program consisting of progressive strengthening exercises, Russian current stimulation, and Kinesio taping. Significant clinical improvement was observed following rehabilitation. Pain intensity on the Numeric Pain Rating Scale reduced from 8–9/10 to 2–3/10, while the Kujala Anterior Knee Pain Scale improved from 58/100 to 91/100. Functional activities such as stair climbing, prolonged sitting, and exercise performance improved considerably. A structured physiotherapy rehabilitation program incorporating progressive strengthening, Russian current stimulation, and Kinesio taping was effective in reducing pain, improving functional ability, and restoring muscle balance in a young adult with chondromalacia patella and Grade II meniscal tear. Conservative physiotherapy management may serve as an effective first-line treatment approach in similar cases.
