Impact of Occupational Therapy on Wheelchair Propulsion in Paraplegia

Abstract Patients with paraplegia rely exclusively on their upper extremities for mobility, transfers, and daily activities, due to impaired lower limb function. Manual wheelchair users experience increased demand on their arms and hands, often resulting in overuse, fatigue, and decreased independence. Occupational therapy with the Baltimore Therapeutic Equipment (BTE) Primus RS, a computerized strengthening device, offers structured training for the shoulder, elbow, wrist, and hand. Despite its clinical value, no studies have evaluated therapeutic interventions using the BTE Primus RS to enhance wheelchair propulsion in paraplegic individuals. This study employed a single-group pre-test/post-test design. Fifteen participants with traumatic paraplegia completed 30 sessions over 6 weeks. The Wheelchair Propulsion Test Version 1.0 (WPT-1.0) assessed outcomes. Results demonstrated significant improvements in propulsion: increased speed, reduced push frequency, and enhanced propulsion effectiveness. All changes reached statistical significance. Occupational therapy with the BTE Primus RS yielded significant, clinically relevant improvements in wheelchair propulsion among individuals with paraplegia, promoting greater independence and more efficient mobility, thereby supporting its rehabilitative role.
