Abstract This study examines healthcare access barriers and utilisation patterns among the ailing population in Imphal, Manipur, using a cross-sectional survey of 500 respondents (50.8% males and 49.2% females). The findings reveal a substantial burden of chronic illness, with 79.8% of respondents reporting at least one chronic disease. Diabetes (29.8%) was the most prevalent condition, followed by arthritis (20.2%), respiratory diseases (14.2%), and heart disease (14.2%). Major barriers to healthcare access included the high cost of treatment and medication (82.8%), financial constraints (75.0%), inadequate care and services (52.4%), and geographical and transportation difficulties (43.2%). Healthcare utilisation remained low, with 84.0% of respondents rarely visiting healthcare facilities. A strong preference for private healthcare was observed, with 83.8% seeking treatment in private clinics and 8.8% in government hospitals. Respondents rated private facilities significantly higher in cleanliness, equipment adequacy, availability of medicines, staff attitude, and specialist services. The study highlights the need to strengthen public healthcare infrastructure, reduce financial barriers, and improve access to healthcare in Imphal.
