Return To Meaningful Occupation Through Occupational Therapy in Carpal Tunnel Syndrome: A Case Report

| Abstract | ABSTRACT Background Carpal tunnel syndrome (CTS) is the most common compressive neuropathy of the upper limb and can result in pain, sensory deficits, reduced hand function, and limitations in activities of daily living. Occupational therapy plays an important role in facilitating recovery following surgical intervention. Case presentation A 51-year-old housewife with a five-year history of bilateral(B/L) CTS predominantly affecting the right hand underwent carpal tunnel release. Following surgery, she experienced pain, restricted wrist and thumb movements, sensory impairment in around the surgical site, reduced hand function, and difficulty performing self-care and household activities. She was referred for occupational therapy on 20 April 2023. Intervention A structured occupational therapy hand rehabilitation program was provided for 16 weeks, four sessions per week, along with a home program. The intervention included positioning, range of motion exercise, tendon and median nerve gliding exercise, sensory reeducation, strengthening exercise, activity of daily living, work conditioning, and household task simulation. Outcome Following intervention, pain decreased, wrist and thumb range of motion improved, and functional independence increased. The readiness for return-to-work scale indicated improved readiness to resume the occupational role. The client successfully resumed household activities, including cooking, cleaning, and handling household objects. Conclusion This case report highlights the effectiveness of occupational therapy in improving hand function, independence in the activities of daily living, and return to meaningful occupation following complex Carpal tunnel syndrome. |
