Therapeutic Role of Kshar in Visha Chikitsa: Ayurveda and Modern Review

| Abstract | Background: Kshar (alkaline plant-ash preparations) represents a potent therapeutic category in Ayurvedic pharmacology. Classical treatises document three principal types—Yava Kshar (potassium carbonate-rich), Sarji Kshar (sodium carbonate), and Palash Kshar—for managing envenomation (Visha Chikitsa) from snakes, scorpions, rodents, insects, and plant toxins. Methods: Following PRISMA guidelines, this comprehensive review collates classical references, formulations, routes of administration, and indications of Kshar in toxicology. These traditional applications are then correlated with contemporary biochemical and toxicological evidence derived from analytical, preclinical, and clinical studies. Results: Classical applications of Kshar are driven by its Tikshna, Ushna, Krimighna, and Vishaghna properties. Modern preclinical investigations validate its multifaceted role, demonstrating significant anti-inflammatory, antiprotease, phospholipase A2 (PLA2) inhibitory, antimicrobial, and emetic activities. Additionally, pilot trials and observational studies support its adjunctive efficacy in snakebite and scorpion sting management. The specific alkaline pH, mineral composition, and electrolyte profile of these alkalis closely align with modern toxicological mechanisms, including venom enzyme inhibition, bioavailability enhancement, and urinary alkalinization for systemic toxin clearance. Conclusion: The validated pharmacological actions of Kshar provide a robust scientific rationale for its traditional use in poison management. To fully integrate these alkaline preparations into contemporary clinical toxicology, systematic multi-centre randomized controlled trials and molecular-level validations of Kshar-based antitoxic formulations are essential. |
