Emerging Trends in Antibiotic Resistance Among Uropathogens Causing Urinary Tract Infections in India: A Comprehensive Review of Recent Epidemiological Data

Abstract Urinary tract infections (UTIs) represent one of the most common bacterial infections globally, with a particularly high burden in India due to factors such as overcrowding, poor sanitation, and widespread antibiotic misuse. This review synthesises recent data on the prevalence, common pathogens, and antibiotic resistance patterns of uropathogens in Indian settings, with a focus on community-acquired and hospital-associated cases. Escherichia coli remains the predominant isolate (typically 50-70%), followed by Klebsiella pneumoniae (15-25%) and Pseudomonas aeruginosa. High rates of resistance to first-line agents like fluoroquinolones, third-generation cephalosporins, and cotrimoxazole are observed, alongside rising carbapenem resistance (often exceeding 20% in E. coli and K. pneumoniae in community settings). Extended-spectrum beta-lactamase (ESBL) production is prevalent (30-60% in many studies). Enhanced surveillance, stewardship programs, and development of region-specific guidelines are urgently needed.
