Case Report: “Recurrent Erosive Oral Lichen Planus: Clinical Presentation, Management, and Follow-Up”

Abstract ABSTRACT
Aim and Background:
Oral lichen planus (OLP) is a chronic inflammatory mucocutaneous disorder with premalignant potential. The erosive form is symptomatic and prone to recurrence. This case highlights recurrent erosive OLP in a 60-year-old male.
Case Description:
A 60-year-old male presented with pain in the upper right posterior teeth for 2–3 days and oral ulceration of 15 days duration. He reported recurrent ulcerations and a history of gutkha chewing, discontinued 10 years ago. Intraoral examination revealed bilateral erythematous, ulcerated buccal mucosa with leukoplakic patches and superimposed candidiasis A biopsy in 2023 showed hyperplastic squamous epithelium with features of leukoplakia and OLP, confirming recurrence. The patient was managed with systemic and topical corticosteroids, antifungal therapy, analgesics, chlorhexidine mouthwash, and symptomatic treatment of involved teeth. He was counseled for tobacco cessation and advised regular follow-up. After 15 days, partial symptomatic relief was noted with reduction in burning sensation and ulceration.
Conclusion:
Recurrent erosive OLP requires early diagnosis, comprehensive management, and strict follow-up due to its malignant potential.
Clinical Significance:
This case emphasizes the importance of habit cessation, combined medical and dental management, and long-term monitoring in patients with erosive OLP and associated dental conditions to minimize recurrence and prevent malignant transformation.
