Frailty Rehabilitation Programs: Current Evidence, Clinical Applications, and Future Perspectives

| Abstract | Frailty is a multidimensional geriatric syndrome characterized by decreased physiological reserve, diminished resilience to stressors, and increased vulnerability to adverse health outcomes. The global rise in the aging population has substantially increased the prevalence of frailty, making it a major public health concern. Frail older adults are at heightened risk of falls, disability, hospitalization, institutionalization, and mortality, leading to significant healthcare utilization and economic burden. Rehabilitation plays a pivotal role in mitigating the progression of frailty by improving physical performance, functional independence, psychological well-being, and quality of life. Contemporary rehabilitation programs have evolved from isolated exercise interventions to comprehensive multidisciplinary strategies integrating resistance training, aerobic conditioning, balance exercises, nutritional optimization, cognitive stimulation, psychosocial support, and technology-assisted monitoring. This review summarizes current evidence regarding frailty rehabilitation programs, emphasizing their clinical effectiveness, essential components, assessment methods, and implementation across hospital, outpatient, community, and home-based settings. Recent advances involving wearable sensors, tele-rehabilitation, artificial intelligence-assisted monitoring, and digital health technologies are also discussed as emerging approaches to improve accessibility and personalized rehabilitation. Evidence from recent clinical studies consistently demonstrates that structured, individualized, and multidisciplinary rehabilitation interventions significantly enhance muscle strength, gait speed, balance, functional mobility, activities of daily living, and health-related quality of life while reducing fall risk and hospitalization among frail older adults. Despite these positive outcomes, several challenges remain, including limited accessibility, poor long-term adherence, workforce shortages, and variability in rehabilitation protocols. Future research should focus on standardized intervention protocols, long-term outcome evaluation, cost-effectiveness, and precision rehabilitation strategies that integrate digital technologies with patient-centered care. Comprehensive frailty rehabilitation programs represent one of the most effective non-pharmacological strategies for promoting healthy aging and maintaining independence among older adults. |
