Strengthening Immunisation to Achieve TB Mukt Bharat

Abstract Childhood tuberculosis remains a significant public health challenge in India despite high BCG vaccination coverage. Routine immunisation services and tuberculosis control programmes continue to function largely as parallel systems, resulting in missed opportunities for early identification of children exposed to tuberculosis.

Objective: To explore how integrating routine immunisation services with tuberculosis preventive strategies, including household contact screening and tuberculosis preventive treatment (TPT), can strengthen childhood TB prevention.

Methods: This article presents a policy and practice perspective based on current national guidelines, published evidence, and operational experience in paediatric tuberculosis. It proposes practical, low-cost approaches to integrate immunisation services with the National TB Elimination Programme.

Results: Routine immunisation provides repeated contact with almost every child during early life, creating opportunities to identify household TB exposure, facilitate timely referral, and improve uptake of TPT. Simple modifications to immunisation counselling and referral pathways can strengthen collaboration between immunisation and TB programmes without substantial additional resources.

Conclusion: Integrating immunisation services with tuberculosis prevention represents a feasible and scalable strategy to reduce childhood tuberculosis and accelerate progress towards TB Mukt Bharat and India's goal of TB elimination by 2035.
