Phenotypic Improvement in Polyendocrine Metabolic Ovarian Syndrome Following Comprehensive Unani Management: A Case Report

Background: Polyendocrine Metabolic Ovarian Syndrome (PMOS), formerly known as Polycystic Ovary Syndrome (PCOS), is increasingly recognized as a complex multisystem disorder characterized by reproductive, endocrine and metabolic dysfunction. Although conventional treatment can effectively manage individual manifestations, achieving concurrent improvement across multiple disease domains remains challenging. Clinical evidence documenting such comprehensive improvement following Unani management is limited. This case report describes the outcome of a comprehensive phase-wise Unani therapeutic approach in a young woman with PMOS.
Case Summary: A 21-year-old unmarried woman presented with a five-year history of irregular menstrual cycles, excessive facial hair growth and progressive weight gain. Clinical examination, biochemical and hormonal investigations and ultrasonographic findings fulfilled the revised Rotterdam diagnostic criteria for PMOS. The patient received a five-month comprehensive phase-wise Unani treatment protocol comprising Munzij, Musaffi-e-Khoon, Mushil, Tabreed, maintenance pharmacotherapy, Naura, dietary regulation, and lifestyle modification, with regular clinical, laboratory and radiological follow-up.
Results: The patient demonstrated progressive restoration of menstrual cyclicity from one cycle every 3–4 months to regular 30–32-day intervals. Improvement was also observed in hirsutism, acanthosis nigricans, body weight, body mass index, hormonal profile, lipid profile, and bilateral ovarian morphology on ultrasonography. Liver and renal function remained within normal limits throughout treatment and no treatment-related adverse effects were reported. Collectively, these findings indicated objective improvement across clinical, anthropometric, hormonal, metabolic and radiological domains with a favourable safety profile.

Conclusion: This case highlights the potential of a comprehensive phase-wise Unani therapeutic approach in the management of PMOS. The concurrent improvement observed across multiple objective outcome domains suggests that this holistic strategy may offer clinically meaningful benefits while maintaining good tolerability. Although conclusions regarding efficacy cannot be drawn from a single case, these findings support further well-designed prospective controlled studies to validate the role of comprehensive Unani management in PMOS.
