Abstract Patellofemoral Pain Syndrome (PFPS) is a common cause of anterior knee pain among runners and is often associated with altered lower-limb biomechanics. Weakness of the gluteus maximus and gluteus medius muscles may impair hip stability, increase patellofemoral joint stress, and reduce knee function. Although gluteal muscle dysfunction has been implicated in PFPS, the relationship between gluteal muscle strength and functional knee outcomes in runners remains inadequately understood. Therefore, this study aimed to investigate the correlation between gluteus maximus and gluteus medius muscle strength and anterior knee pain in runners with PFPS to support evidence-based rehabilitation strategies.
