Regulating for Access: A Policy Evaluation of India's Drug Price Control Regime and Its Trade-Offs Between Affordability and Availability in Cardiovascular Care

| Abstract | Abstract With cardiovascular conditions persisting as a primary driver of mortality across India, the provision of low-cost therapeutics has emerged as an essential public health imperative. Consequently, the pharmaceutical landscape has been fundamentally altered by state-mandated price ceilings. While these regulatory interventions are frequently lauded for lowering out-of-pocket expenses, the degree to which they dictate medicine availability and the substitution dynamics among medical practitioners, retailers, and consumers—particularly within semi-urban locales—remains under-researched. This inquiry explores the repercussions of National Pharmaceutical Pricing Authority (NPPA) interventions on the supply of cardiac treatments and the subsequent behavioral adjustments of participants throughout the distribution network. The research posits that while price regulation has successfully optimized patient affordability, it has concurrently triggered secondary effects by reshaping inventory management, clinical prescribing habits, and brand-switching behaviors, thereby complicating reliable access to vital therapies. Employing a mixed-methods framework, the investigation synthesizes quantitative patient surveys, qualitative stakeholder interviews, detailed corporate case analyses, and secondary policy data to provide a holistic assessment of regulatory impacts. By triangulating these multifaceted datasets, the study provides a comprehensive perspective on how pricing mandates influence stakeholder decision-making and supply chain stability. Ultimately, this work enriches the discourse on health equity and pharmaceutical regulation by broadening the analysis beyond simple cost metrics to encompass market resilience and stakeholder behavior, offering critical evidence to inform the development of sustainable pricing frameworks. |
