Painless Dentistry: Contemporary Concepts And Emerging Approaches To Dental Pain Management

| Abstract | Dental pain is a major cause of fear, anxiety, and avoidance of dental treatment. Pain perception is influenced by a complex interplay of biological, psychological, and behavioral factors. This review summarizes the mechanisms and theories of dental pain, factors influencing pain perception, local anaesthesia, pharmacological pain management, and emerging techniques for painless dentistry. Dental pain is primarily mediated by Aδ and C fibers through the trigeminal system and involves the processes of transduction, transmission, modulation, and perception. Effective local anaesthesia remains the cornerstone of dental pain control, although supplementary techniques may be required in challenging clinical situations. Pharmacological agents, particularly non-steroidal anti-inflammatory drugs (NSAIDs) and acetaminophen, are commonly used for postoperative pain management. Emerging approaches, including computer-controlled local anaesthesia delivery, vibrotactile stimulation, hypnosis, acupuncture, cryotherapy, and laser-based analgesia, may further enhance patient comfort and reduce procedural discomfort. Painless dentistry requires a patient-centered, multimodal approach combining effective anaesthesia, anxiety reduction, appropriate analgesic therapy, minimally invasive techniques, and emerging technologies to improve patient comfort, cooperation, and acceptance of dental treatment. |
