Patient counselling in chronic illness : a systemic review

Abstract Patient counseling refers to a process in which pharmacists provide information, advice, and assistance to help patients use their medications appropriately. It involves both verbal and written communication tailored to the patient’s individual needs. Pharmacist counselling is an important service that has been associated with improved outcomes. It should be viewed as one component of the overall drug-use process. Pharmacists counseling patients prescribed long-term medications need to understand the lifestyle effects of chronic illness, particularly the different types of "work" the chronically ill patient must undertake. Medication Therapy Management (MTM): “Services provided by pharmacists that improve treatment outcomes. These services promote the safe and effective use of medications. Ultimately, MTM is a way to provide better care for patients
