Selection of Occlusal Philosophy and Scheme for Full Mouth Rehabilitation: A Comprehensive Review

| Abstract | Abstract Objective: This comprehensive review synthesizes current evidence on occlusal philosophies and schemes for full mouth rehabilitation, providing clinicians with a framework for evidence-based decision-making. Methods: The review examines the theoretical foundations and clinical applications of major occlusal philosophies (Gnathological, Pankey-Mann-Schuyler, Hobo twin-stage and Functionalist approaches) and compares the biomechanical and clinical characteristics of principal occlusal schemes (mutually protected occlusion/canine guidance, group function, and balanced occlusion). A literature-based synthesis was conducted to identify key factors guiding the selection of appropriate approaches for individual patients. Results: The analysis reveals that no single occlusal philosophy or scheme is universally applicable. Selection must be individualized based on patient-specific factors including existing occlusal patterns, periodontal status, temporomandibular joint (TMJ) health, degree of tooth wear, presence of parafunctional habits, and clinician expertise. Special considerations for implant-supported rehabilitations and assessment/alteration of vertical dimension of occlusion (VDO) are addressed. Conclusion: Evidence-based recommendations emphasize thorough diagnostic evaluation, provisional testing of proposed occlusal changes, and systematic implementation of the chosen philosophy to achieve predictable functional and esthetic outcomes. Successful full mouth rehabilitation depends on individualized treatment planning rather than rigid adherence to any single approach.
